I / the participant warrant that the information supplied aforesaid is true, correct and complete. I am/ the participant is the participate of the Activities/ Courses/ Training and I declare that I / the participant can swim with clothes for at least 50 meters; and I / the participant fulfill all the Activities/ Courses/ Training entry requirement. I am / the participant is healthy, physically fit, and suitable to participate in dragon boat training course. I am / the participant is willing to comply with all the rules and regulations setup by the organizer. I / the participant understand that join the Activities/ Courses/ Training at my/ his/ her own risk. I / the participant also agree to discharge the organizer from any responsibility in the event of injury, death or loss of property incurred, as a consequence of or while traveling to and from the venue(s) if the cause of injury, death or loss of property is due to my/ his/ her own negligence or inadequacy in health and fitness. Applicant aged under 18 should obtain consent from his / her parent / guardian to submit this application and the above-mentioned declaration and regulations.
The organizer has purchased Public Liability and Personal Incident Insurance packages for accidental death respectively. I will purchase extra insurance coverage myself if necessary.
According to Personal Data (Privacy) Ordinance, the information provided by the applicant(s) will only be used for the purposes of enrolment and promotion of recreation and sports activities organized by the organizer and co-organizing parties. For correction of or access the personal data collected by means of this form, I/ the participant can contact the organization.
I / the participant also grant the right to the organizer to film video and take photographs of me / participant in the registered activities. I / the participant understand that the organizer will use photos and video clips from the activities for marketing, promotional and educational purpose.
